**The Case Management Assistant responsibilities include:**
+ Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
+ Responsible for assisting with distributing regulatory documents to identified patients
+ Educating patients about the appeal process related to their discharge
+ Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
+ Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives

+ **Status:** Part time, 16 hours per week
+ **Schedule:** Saturday & Sunday 8:00 AM - 4:30 PM
+ **Weekend Requirement:** Every
+ **On Call:** No
**Education:**
+ Required High School or Equivalent
+ Preferred Associate's
**Experience:**
+ Required 1 year in Medical Related Field
